Obviously if a person performs a deed for which he can be punished by the court (or if he is a Rodef and can be punished at the hands of man), no merit will redeem him. The Iyun Yakov is referring to a person who is supposed to be punished for his sins by Hash-m (e.g. Kares or Misah b'Yedei Shamayim). Sometimes Hash-m will forego the intended punishment due to a person's deed of great merit.
