SUMMARY
1. A person who has an internal wound may not use a Nochri doctor. 2. If one is wounded on the back of his hands or feet, there is a disagreement about whether he may use a Nochri doctor. 3. If one is wounded badly enough that an evaluation is necessary to determine whether he will survive, it is permitted to be Mechalel Shabbos for him. 4. If one has a toothache on Shabbos, he may not gargle with vinegar. 5. One is permitted to use a Nochri doctor who is an expert doctor. 6. One is permitted to manipulate the ear joint on Shabbos. 7. If one is in danger of losing his eye, he may apply medicine on Shabbos.

A BIT MORE
1. An internal wound is considered life-threatening. The Gemara is in doubt about whether a wound of the gums or teeth is considered an internal wound. 2. According to one version of Rebbi Yochanan's statement, whenever a person has a wound which is life-threatening he may not use a Nochri doctor. Since it is permitted to be Mechalel Shabbos for a wound on the back of the hand or foot, it must be a life-threatening wound. Therefore, it is forbidden to use a Nochri doctor. In a second version of his statement, Rebbi Yochanan says it is permitted to use a Nochri doctor for a wound on the back of the hand or foot even though it is life-threatening, since it is not an internal wound. 3. Since he was wounded badly enough that it was not apparent that he would survive, it is considered life-threatening. It is permitted to be Mechalel Shabbos for him. 4. The Rabanan prohibited any medicinal act on Shabbos, lest a person grind herbs to produce the medicine on Shabbos. 5. Even if the Yisrael has an internal wound, he may use an expert Nochri doctor. An expert doctor will be afraid of ruining his reputation. 6. If the ear joint needs to be manipulated, it is considered a life-threatening condition. However, according to one opinion, it is permitted to manipulate the joint only by hand, but not with medicine. Manipulating it by hand is adequate to alleviate the problem. According to another opinion, it is permitted to use medicine, because if it is done by hand it could cause a wound, thereby increasing the Chilul Shabbos. 7. He may even grind the medicine and carry it through a public domain. When an eye is in danger, it is considered a life-threatening condition.
