Malbim: Hashem's arrows are unlike human arrows, which come from the outside to the victim's body. Rather, Hashem takes it from inside his body to the outside. It is an inner illness that Hashem shoots. It strikes his body from inside to outside.

What is "Barak"?

Rashi: It is lightning. Ibn Ezra - so the terror will come on him suddenly.

What do we learn from "mi'Meroraso Yahaloch"?

Rashi: It goes from the bitter fear of the weapon, and from there, there is terror on the Rasha.

Malbim: This 'lightning' does not come from the outside, rather, from his Marah (gall bladder). It is a black Marah (melancholy) that terrifies him. He sees threatening, confounding 'pictures', and dies amidst fear, phobia and insanity.
