Why does it say 'v'Gam Ani"?

Radak: Just like you made Korbanos and Ma'aseros lacking in My house, I made you lack bread and meat in all your cities and residences, Midah k'Neged Midah. Even so, "v'Lo Shavtem Adai"!

Malbim: Also I sounded the Shofar and decreed evil against you. Hashem will sound a Shofar five times ? three times for the decree of hunger, once for plague, once for the sword, and they did not fear and did not repent!

What is "Nikyon Shinayim"?

Rashi citing Targum Yonasan: It is Akhiyos Shinayim (setting the teeth on edge). Radak ? this happens to teeth when they do not eat constantly.

Rashi: It is lack of eating meat, which enters between the teeth ? "ha'Basar Odenu Bein Shineihem" (Bamidbar 11:33).

Radak: It is lack of bread; "v'Choser Lechem" is a repetition. With food, teeth are full; when food lacks, teeth are clean.

4.

Malbim: There is no food, and the teeth are totally clean.

Why does it add "b'Chol Mekomoseichem"?

1.

Malbim: These are small settlements and villages. Kinds of Peros are found there. They will lack bread, for grain, which is used to make bread, will not grow there. Even so, "v'Lo Shavtem Adai"! This is the astonishment "Im Yitaka Shofar ba'Ir v'Am Lo Yecheradu" (3:6).
