Why does it say "Ra'os Rabos v'Lo Sishmor"?
Rashi: Many visions are in front of you, but you do not guard to see My deeds and repent to Me.
Radak: You will see many afflictions, and the eye of your heart will not know why they came. Your eyes see, and they are blind. How does seeing help one who sees, if he does not guard his way?!
Malbim: You can see many things from which you can recognize Hashem's wonders, but you do not guard to investigate and open your eyes.
What is the meaning of "Pako'ach Aznayim v'Lo Yishma"?
Rashi: I engage to open your eyes via My Nevi'im, and none of you hear My words! This is present tense.
Radak: Your ears are open, but not one of you hears the rebuke. It is as if you are deaf! Ra'os and Pako'ach are Makor (like an infinitive). Sishmor and Yishma are singular, for not one of them guards or hears to return to the good path. Sishmor is second person, and Yishma is third person. Many verses are like this, e.g. "Shim'u Amim Kulam" (Melachim I, 22:28).
Malbim: Hashem wants to open the ears of your Navi, and tell him His word, but he does not want to hear!
