Malbim: Due to hunger, there will be rebellion in the city. Some will want to surrender to the Bavliyim, and the soldiers will need to fight the nation.

Rashi: The officers above appointed over punishments. He will destroy them via a plague, via the angel of death, who is among the officers above - "b'Dever Gadol Yamusu" (verse 6).
